One unlucky insurer paid a claim for a building which burnt down after a contractor had left a welding torch on when he popped out for a bite to eat at lunch-time.

A teeny bit careless you might think. But pity the poor insurer that had to pay a second claim for an almost completed roof, which was accidentally burnt down by the same contractor.
